To start with, I am attaching an informed consent form, just below this paragraph. Informed consent is a requirement of the Ontario College of Social Workers and Social Service Workers. I am delivering this program as a Registered Social Worker, so all attendees need to sign and return a PDF copy, together with payment for the program, prior to the start of the first session. The signed consent form, and a cheque e-transfer for payment ($270, the password provided via separate communication) can both be sent to: email@example.com If you prefer, you can bring a physical copy of the signed consent, with a cheque or cash, to our first meeting.
Rhea Plosker MASc, MC, RSW, RP, PMP Rhea is an Engineer, Registered Social Worker, and Registered Psychotherapist. Social Work is Rhea’s second career, pursued after more than two decades of working in technology leadership roles in international organizations. Rhea also built an independent consulting practice, and continues her work as a project and change management consultant and trainer. Rhea has worked on several agile projects, so has a particular interest in Intelliware's focus.She is also an executive and career coach, working privately and as an affiliate with the Canadian Centre for Brief Coaching at the University of Toronto. Rhea started her mindfulness journey through taking a program to support her own chronic health issue and is certified to facilitate Mindfulness Based Cognitive Therapy programs. Rhea is on the faculty at the Centre for Mindfulness Studies in Toronto where she facilitates community and online programs, and is the faculty lead supporting the expansion and development of online programming.
